I used http://www.thestampmaker.com/ for my address stamp which came out great. I didn't get a custom one, but they do custom ones with crazy fast turnaround.

i bought a custom rubber stamp for personal use (address) from >http://www.canville.net/stamps/

i was quite happy with it and the price.
