Okay, first I think toothbrushes should be thrown away and replaced regularly, so I wouldn't do anything that would take a lot of time.

How about melting the handle and warping it while soft into sinuous shapes? Don't get it too hot, and don't make the stretches too thin, because it will make it brittle. But a Dali-esque squiggle in a clear glass on the sink could be cool.

Alternately you could see if you could find the kind that's just the brush part, without handle. They usually do this for electric toothbrushes, but sometimes they have just ordinary ones at the drugstore. Then you can decorate the handle, and keep it for a long time.
